Nominate an Honoree

A meal that honors a life...

while they are still here to feel the love.

At Last Supper Foundation, we create story-driven meals for individuals living with terminal illness; gatherings that honor a life through food, memory, and shared presence.
If someone you love is approaching the end of life and would find meaning in a meal

that reflects their story, you may nominate them here.
This offering is not about celebration or farewell alone. It is about being together, remembering what has mattered, and nourishing connection in a tender season

Who This Is For

An honoree may be:

An individual living with terminal illness

Someone who finds comfort in reflection, storytelling, or shared meals

A person who would appreciate a small, intimate gathering with loved ones

The honoree does not need to be a “food person.” Many individuals living with terminal illness lose their appetite.

What matters most is that the meal feels true to who they are.

What to Expect

From first conversation to gathering,

here's what the journey looks like.

1

A Listening Call

We begin with a warm, spacious conversation with the honoree (and family, if desired). This is where we hear stories, memories, favorite foods, and the threads that have shaped their life.

2

Menu and Story Design

Our team translates these stories into a four-course meal: each course reflecting a chapter of the honoree's journey.

3

Preparing the Meal

Chefs, volunteers, and loved ones come together to prepare the dinner with deep attention, care, and presence.

4

The Gathering

Friends and family come together for an evening of storytelling, connection, and celebration... a chance to honor a life in a way that is felt, remembered, and shared.

There is no cost to the honoree or their family.

Each meal is offered as an act of service and love, made possible by donors, volunteers, and community partners.

What This Costs

The Nomination Process

You can nominate someone if:

You are a loved one, caregiver, family member, or friend

You are the honoree themselves

You are a healthcare provider or spiritual counselor who feels this would be supportive

After you submit a nomination:

We will reach out within 5-7 days

We schedule the initial listening call

Together, we explore whether this is the right moment for a Last Supper

This process is always gentle, always at the honoree's pace, and always rooted in compassion.

Ready to Begin?

Nominate an honoree below.

Please share whatever you feel comfortable sharing. If some details are unclear, that is completely okay...

we will walk with you through the process.

About the Honoree:

A Last Supper is not an ending...

it is a celebration of a life lived.

It is chance to say what needs to be said,

to remember what is beautiful,

and to surround someone with the people who love them most.

We are here to help you create that moment.
